Who Am I: Assessment, ISP & Change of Condition

Duration: 6 Hours
Designed for staff who complete Individual Service Plans (ISP) or other care plans and covers CBRF, RCAC and AFH requirements. Learn how to complete pre-assessments, create care plans and respond to changes in condition. Experience interactive, real-life examples and takeaway best practice tools for care plans.
