What to Do Prior to Meeting

Complete these intake forms in advance and bring those to your appointment.

VITA INTAKE FORMS 

What to Bring

These required documents enable our VITA volunteers to prepare your taxes. Have ready your photo identification, social security cards, completed intake forms, prior year tax return, wage and tax statements, and any other information — like a voided check to direct deposit the tax refund. If you choose to meet virtually, you need to scan the documents to one PDF. See details below.

Identification Information

  • Proof of identification (Government-issued photo ID) for you and your spouse.
  • Social Security cards for you, your spouse, and all dependents.

Prior Tax Information

  • A complete copy of last year's federal and state income tax returns (not required, but helpful).
  • A record of any estimated tax payments made for last year.

Current Year Tax Information

  • Wage statements - Form(s) W-2.
  • Proof of health insurance coverage - Form 1095A/B/C.
  • Unemployment compensation - Form 1099-G.
  • Interest and dividend statements - Form(s) 1099 INT and 1099 DIV.
  • Pension and social security statements - Form(s) 1099 R and SSA-1099.
  • Tuition payment statements - Form(s) 1098-T.
  • Property tax receipts for any property taxes paid in 2024.

Other Information

  • Information regarding any other sources of income, including State and Federal SSI and SSD payments, and income reported on Form 1099 MISC.
  • If you may be able to claim the Homestead Credit from the State of Wisconsin:
    • A completed rent certificate (or certificates) and/or copy of your 2024 property tax bill.
    • A printout of any Wisconsin Work (W2) payments.
    • Proof of the amount of child support and/or alimony received.
  • If direct deposit of your tax refund is desired, a voided check for a checking account or deposit slip for a savings account.

What We Assist With

We can help with most basic tax returns, but there are some we don't support.

Federal Returns

  • Forms 1040 and 1040-SR.
  • Form 8863 (Education Credit).
  • Schedule EIC (Earned Income Credit).
  • Form 2441 (Child and Dependent Care Credit).
  • Form 8812 (Additional Child Tax Credit).

Wisconsin Returns

  • Forms 1.
  • Schedule H (Homestead Credit).

What We Can't Assist With

  • Schedule A (Medical expenses, state and local taxes, charitable contributions, etc.).
  • Schedule C (Profit or loss from a business).
  • Schedule D (Gains and losses from sales of stocks, bonds, and other property).
  • Schedule E (Income from rental property or partnership).
  • Schedule F (Farm income).
  • Married taxpayers who file separate tax returns.

Tax Assistance Location & Times

The UW-Green Bay Volunteer Income Tax Assistance program help prepare tax returns each year from January through April.  In 2025, appointments will be available from January 27 through April 30.

Where to Park

Those who meet with us in person should park in the Wood Hall Lot on the UW-Green Bay campus.

Directions

  1. From Nicolet Drive, enter campus via the Nicolet Entrance or Main Entrance.
  2. Turn onto Wood Hall Drive
  3. Continue until you arrive at the Wood Hall Lot
  4. Park and walk east to the nearest academic building, L.G. Wood Hall.
  5. Find the Center for Financial Wellness on the first floor.

AARP Tax Help

Despite the name, AARP (American Association of Retired Persons) Foundation Tax Aide has no age, income or AARP Membership requirement. Volunteers are available nationwide and are IRS-certified each year, and they can assist you with taxes in-person, low-contact or contact-free.
