Enter Survey Data

There are three versions of the Wisconsin Shorebird Survey Form:

• Online form
1. Our first choice as data are entered automatically into our database.
2. If you have a problem (form won't submit or you receive an error message), please contact Jennifer Davis (920-465-2545 / davisj@uwgb.edu) and describe the problem you are having.

• Microsoft Word form
1. Save the file to your computer.
2. Open the file in Microsoft Word 2003 and fill in the data fields.
3. Save the completed file.
4. Email the file to Jennifer Davis (davisj@uwgb.edu).

• PDF form
1. Save the file to your computer.
2. Open the file using Adobe Reader (available for free from Adobe.com).
3. Print the form.
4. Fill in your survey information.
5. Send the completed form to Jennifer Davis, University of Wisconsin-Green Bay, Cofrin Center for Biodiversity, Green Bay, WI 54311.
